Prior approval is extremely difficult in a NICU when the neonates cannot wait 1-2 days to start being fed unfortunately.

Erin - if you have a patient on a commercial plan, offer PDHM to baby and submit a PA so that you can get reimbursed. The effective date is when the PA was written.
